IMPORTANT NOTICE FOR MEDICARE PATIENTS

Patients enrolled in Medicare generally cannot elect to use our self-pay pricing for services covered by Medicare. Loria Mobile Medicine bills Medicare for covered services in accordance with Medicare regulations, and patients are responsible only for applicable deductibles, coinsurance, and any other amounts permitted under Medicare rules. However, services that Medicare does not cover may be available for self-pay. Patients will be informed of applicable charges before receiving these services.

YOUR RIGHT TO A GOOD FAITH ESTIMATE

Under the federal No Surprises Act, patients who do not have health insurance or who choose not to use their insurance have the right to receive a Good Faith Estimate explaining the expected cost of their medical care. You have the right to receive a written Good Faith Estimate:

  • At least 1 business day before your appointment if scheduled 3–9 business days in advance.

  • At least 3 business days before your appointment if scheduled 10 or more business days in advance.

  • Upon request, even if you have not scheduled an appointment.

Your estimate will include the expected charges for services reasonably anticipated at the time of scheduling. Actual charges may differ if additional services become medically necessary. If you receive a bill that is at least $400 more than your Good Faith Estimate, you may be eligible to dispute the bill through the federal patient-provider dispute resolution process. To request a Good Faith Estimate, please contact Loria Mobile Medicine before your appointment.
